Do you take my dental insurance?
The office is not in-network with many insurance plans due to limitations those plans place on treatment recommendations. As a courtesy, the office will file insurance claims on your behalf, but your policy is a contract between you and your insurance company and you remain responsible for all charges.
What payment options are available?
Payment is due at the time services are rendered unless prior arrangements have been made. The office accepts cash, check, and credit cards, and offers a 5% courtesy discount for payment in full by cash or check, phased payment by appointment, 3-month payment plans with a card on file, and extended financing through CareCredit.
What are your office hours?
The office is open Monday through Thursday from 8:00 AM to 4:30 PM and Friday from 8:00 AM to 1:00 PM. It is closed Saturday and Sunday.
Do you offer Invisalign?
Yes. Invisalign uses custom made clear plastic aligners instead of metal braces to move your teeth back into place. Because the aligners are removable, they are easier to keep clean and you can brush, floss, and eat normally.

What do I need to do before my first visit?
New patient registration and privacy forms can be downloaded from the practice website and completed at home. You can fax the printed and completed forms in or bring them with you to your visit.
