Midtown Chiropractic & Physical Therapy is an integrated musculoskeletal clinic on West Belmont Avenue in Chicago's Lakeview and Roscoe Village neighborhood, offering chiropractic care, physical therapy, and clinical massage therapy under one roof. The clinic takes a conservative, non-surgical approach aimed at identifying the root cause of a condition rather than treating symptoms alone, and its chiropractors, physical therapists, and licensed massage therapists communicate with one another so each patient receives coordinated care. Treatment draws on the McKenzie Method of Mechanical Diagnosis and Therapy, Active Release Technique, dry needling, deep tissue and neuromuscular massage, cupping therapy, and corrective exercise. Patients are seen for back and neck pain, sciatica, shoulder and knee pain, tennis elbow, whiplash and auto accident injuries, sprains and strains, sports and overuse injuries, post-surgical rehabilitation, posture problems, and chronic pain. Physical therapy sessions are one-on-one rather than shared across a room of patients, and no referral from another doctor is required to begin treatment.

What are your hours?
The clinic is open Monday through Thursday from 9:00 AM to 7:00 PM, Friday from 9:00 AM to 5:00 PM, and Saturday from 9:00 AM to 2:00 PM. It is closed on Sunday.
What services do you offer?
We provide chiropractic care, physical therapy, and clinical massage therapy. Treatment can include Active Release Technique, dry needling, the McKenzie Method, deep tissue and neuromuscular massage, myofascial release, lymphatic drainage, sports massage, prenatal and postnatal massage, and cupping therapy.
Do I need a referral to be seen?
No. You do not need a referral from another doctor to get treatment with us. You can book an appointment directly.
Do you take insurance?
The clinic is out of network with all insurance providers except BlueCross BlueShield PPO plans. BlueCross BlueShield determines all pricing, not the clinic, and we offer a complimentary check of your coverage under a BCBS PPO plan. Patients without BCBS PPO coverage can use our published self-pay rates.
What do visits cost if I am paying myself?
Self-pay rates for patients not using insurance are $250 for a new patient consultation, $100 for a 30 minute treatment, and $175 for a 60 minute treatment. Dry needling therapy is a $30 add-on charged in addition to your visit cost. Massage therapy is $110 for 60 minutes and $165 for 90 minutes, with 90 minute massages offered by availability only.
What conditions do you treat?
We treat back and neck pain, sciatica and nerve-related pain, shoulder and knee pain, tennis elbow, whiplash and auto accident injuries, sprains and strains, sports and overuse injuries, workplace injuries, post-surgical rehabilitation, posture problems, and chronic pain.
What is your cancellation policy?
The clinic asks for at least 24 hours notice to cancel or reschedule. If an appointment is canceled within 24 hours of the scheduled time, or you do not attend without prior notice, you will be charged the full cost of the appointment. Emergencies are considered case by case.
